If you are a new patient to our office, you will need to fill out a New Patient Registration form with your dental insurance information ( if you have one) and medical history form. Please make sure you arrive at least 15 minutes earlier than your scheduled appointment and bring a photo ID and your dental insurance card.
Dr. Vazira will review your medical and dental history, and will take x-rays (when needed). She will perform a comprehensive exam and discuss all findings and present all options for dental treatment.
